Integrated Case Management Services (ICMS)

Population Served: Severely and persistently mentally ill adults (ages 18 and over) residing in Middlesex County who have a history of hospitalization at the State Psychiatric facility..
Referral Source: State or County Psychiatric Hospitals, Short Term Care Facilities (STCF) and Aging-In Agencies
Funding Source: New Jersey Division of Mental Health Services
Number Served: 510 at any one time.
Service Provided: Outreach, linkage to mental health and other supportive services that are geared towards collaborative discharge planning from hospitals and preventing re-hospitalization while optimizing success in community settings.
Length of Service: Approximately 18 months

Homeless Case Management (PATH and TH)
PATH
Population Served: Severely and persistently mentally ill adults (ages 18 and over) who are homeless or at risk of homelessness in Middlesex County.
Referral Source: Referrals are accepted from outpatient or inpatient providers, social service agencies, homeless shelters, soup kitchens; individuals seeking service can refer themselves as well. Particular emphasis is placed upon those without mental health services.
Funding Source: New Jersey Division of Mental Health Services
Number Served: 30 active cases per year; and 100 referrals only.
Service Provided: Outreach, linkage to mental health and other supportive services that are geared towards the maintenance of housing and acquisition of independent living skills while optimizing success in community settings.
Length of Service: Approximately 6 months after housing and mental health linkage are successful.

Transitional Housing (TH)
Population Served: Severely and persistently mentally ill adults (ages 18 and over) who are homeless (homeless is defined as "on the street or in shelters") in Middlesex County.
Referral Source: Referrals the same as PATH. Eligibility is exclusively currently homeless.
Funding Source: HUD
Number Served: 6-7 ongoing
Service Provided: Participants live in designated apartments and pay 1/3 income towards rent. Staff and clients work together to develop necessary skills to develop progressive self sufficiency. Upon graduation, individuals with adequate independent living skills are eligible to receive Section 8 vouchers and will receive assistance in locating appropriate placement.

Intensive Family Support Services (IFSS)

Population Served: Family members of mentally ill adults (mentally ill member must be 18 or over) residing in Middlesex County.
Referral Source: Any source, families or individuals seeking service can refer themselves; referrals are also accepted from outpatient or inpatient providers and other social service agencies.
Funding Source: New Jersey Division of Mental Health Services
Number Served: 60 new families per year, total of 130 families receiving various services per year.
Service Provided: Family support, advocacy and education, multi-family groups, educational conferences, referral and linkage.
Length of Service: Depends on individual case needs and preferences.

Youth Case Management Services (YCM)

Population Served: Severely Emotionally Disturbed Youth (ages 18 and under) residing in Middlesex County.

Number Served: 75 cases per year, plus 100 referrals
Service Provided: Outreach, linkage to mental health and other supportive services that are geared towards the maintenance of housing while optimizing success in community settings.

Geriatric Case Management Services

Population Served: Residents of New Brunswick Housing Authority senior residences ONLY.
Referral Source: Individuals seeking service can refer themselves; referrals are also accepted from outpatient or inpatient providers and NBHA staff. Particular emphasis is placed upon those with concrete mental health and medical care needs.
Service Provided: Linkage to medical, psychiatric and social services, recreational socialization, family services, needs assessment.
Funding Source: The New Brunswick Housing Authority

Specialized Case Management and Outreach Services (SCMOS)

Population Served: Mentally ill adults involved in the correctional system or the elderly who are unengaged and needing mental health services in Middlesex County.
Referral Source: Individuals or families seeking service for an eligible individual can make referrals. Referrals are also accepted from outpatient or inpatient mental health providers, social service agencies or correctional systems.
Funding Source: The County of Middlesex
Service Provided: Outreach services are geared towards successful linkage to mental health and other supportive social services.
Length of Service: Approximately 30 days.

One Time Financial Housing Assistance

Population Served: Married or single severely and persistently mentally ill adults (ages 18 and over) residing in Middlesex County.
Referral Source: Application must be completed by a mental health professional who is working with the client. No applications accepted directly from clients.
Service Provided: Funds are available to support an independent living arrangement for individuals who would otherwise be unable to get temporary or long term housing due to the economic exigencies of their mental health care. Clients shall be able to provide evidence of their ability to sustain independent living arrangement. Financial assistance which can be used for the exact payment of rent, security deposit, mortgage up to, but not to exceed, $2,000.00 for any one client OR essential furnishings to include table, chair and bed only. Payment is made directly to housing or furnishing provider.

Population Served: Former residents of Memorial Homes in New Brunswick, public housing residents of Hope Manor.
Referral Source: New Brunswick Tomorrow
Service Provided: Full case management service; referrals for: childcare, job training, education and transportation. Linkage to medical, psychiatric and social services, recreation, socialization, family services, needs assessment.
