Essex County Advanced Practice Nurse (APN)

Essex County Advanced Practice Nurse (APN) Initiative is funded through the NJ Division of Mental Health, and is housed within Child and Adolescent Outpatient Services.

This initiative provides home-based advanced practice nursing services to youngsters recently discharged from the CCIS unit at Newark Beth Israel Hospital, who might be at high risk for rehospitalization. The APN is assigned to the case upon the child’s discharge from the hospital and follows the child and family until they are connected with an ambulatory care unit.
